Printing Application Form
Print files on A4 size paper. Please DO NOT make any changes. Any changes in the content or format of the forms may result in rejection of the application submitted using that form.
FILLING OF ADMISSION FORM
(A) Duly filled Application form for admission must reach the college office by the prescribed date.
(B) Affix a recent photograph in the appropriate place provided.
(C) Separate forms will have to be filled, in case of candidate wishes to apply for more than One Faculty/Course.
(D) Certificates to be submitted with the form :
1. Highschool (10th) Marksheet ( Photocopy)
2. Highschool (10th) Certificate ( Photocopy)
3. Intermediate (12th) Marksheet ( Photocopy)
4. Intermediate (12th) Certificate ( Phptocopy)
5. Characte/Conduct Certificate ( Orignal)
6. Transfer Certificate ( Orignal)
7. Address Proof (Adhar/Voter Id) ( Photocopy)
Completing and Submitting the Forms
